Transitional Therapy
Whilst many patients may go home from hospital following surgery, for some, additional support from our qualified Chartered Physiotherapists and Occupational Therapist can be an integral part of their recovery. Our transitional therapy programme is designed for patients who are assessed as being medically optimised and ready to leave acute hospital care.
During your inpatient stay, our highly specialist team will complete a full physical and social assessment with you. Following this assessment a treatment plan with goals will be discussed and set jointly with you. All of the specialised therapists are part of the multi-disciplinary team which involves world class Consultants, and excellent standards of care.
Who we see
This period of transition therapy may be beneficial prior to post-surgical inpatient discharge. Our patient centred individualised therapy plans are tailored on an assessment of the individual’s needs.
Patients will have therapy input to enable them to become as self-sufficient as possible. We aim to assist patients to manage their own health conditions and to become independent with their own care during their period of transitional therapy. The nursing team will support them with their fundamental needs should this become necessary.The overall aim is to improve confidence and independence with activities of daily living, and also to achieve a safe method of mobility.
Our service will assess, advise and offer treatment, working towards short-term functional milestones, enabling a timely recovery and return to independence.
Inclusion / exclusion criteria
Inclusion:
- Patients who have appropriate rehabilitation needs which will be determined by a joint physio and OT assessment
Exclusion:
- Patients that have no rehabilitation goals
- Patients that are unable to participate in the rehabilitation process due to the level of cognitive impairment
- Patients that are medically unwell

Transitional Therapy Packages
Transitional Therapy package for one week
- 6 bed nights
- 10 physiotherapy sessions (2 x 30 minute sessions per day, weekends excluded)
- Physiotherapy sessions may include one session of hydrotherapy (as appropriate)
- Up to 6 occupational therapy sessions (weekends excluded)
*Any additional tests or required imaging would be chargeable
Transitional Therapy package for two weeks
- 13 bed nights
- 20 physiotherapy sessions (2 x 30 minute sessions per day, weekends excluded)
- Physiotherapy sessions may include two sessions of hydrotherapy (as appropriate)
- Up to 12 occupational therapy sessions (weekends excluded)
*Any additional tests or required imaging would be chargeable
